Requesting an appointment with Wasatch Foot & Ankle Institute takes just a minute. Fill out the form below with your name, phone number, and a brief description of what's bothering you, and pick the office that's most convenient — Farmington or South Ogden. Our team reviews every request and will call or email you back, usually within one business day, to confirm a time.
If you'd rather speak with someone right away, or your issue needs sooner attention, call the office nearest you directly. New and returning patients are both welcome at either location.
