What is dual syndesmosis TightRope fixation?
The syndesmosis is the strong ligament complex that binds the two lower leg bones — the tibia and fibula — together just above the ankle joint. When this joint is torn or separated, most often from a high ankle sprain or an ankle fracture, the bones can shift out of their normal position and the ankle becomes unstable. Dual syndesmosis TightRope fixation is a surgical technique that uses two flexible, suture-based implants to hold the tibia and fibula in the correct position while the ligaments heal.
Unlike a traditional rigid screw, which locks the bones in a fixed position and is sometimes removed later, the TightRope system allows a small amount of natural motion between the bones as you walk, similar to how the joint moves normally. Using two TightRope devices, rather than one, provides added rotational stability for syndesmosis injuries that need extra support. This procedure is often considered alongside treatment for related injuries such as ankle fractures and lateral ankle instability.
What injuries lead to needing this procedure?
Dual syndesmosis TightRope fixation is used when the syndesmosis ligaments have been torn or the joint has become unstable, most commonly from:
- A high ankle sprain that involves the ligaments above the ankle joint rather than the more common outer-ankle sprain
- An ankle fracture that also disrupts the syndesmosis, such as certain rotational or high-energy injuries
- Sports injuries involving a forceful twisting or outward-rotation mechanism
- Chronic syndesmotic instability that has not resolved with nonsurgical treatment
Because these injuries can be subtle on initial exam, they are sometimes missed or under-treated as a standard ankle sprain, which is why a thorough evaluation matters when a "sprain" is unusually slow to improve.
How do we diagnose a syndesmosis injury?
Diagnosis starts with a physical exam that includes specific stress tests designed to reveal instability between the tibia and fibula, along with a review of how the injury occurred. Because syndesmosis injuries frequently occur alongside ankle fractures or ligament tears, we assess the whole ankle rather than just the area of pain.
Imaging plays a key role in confirming the diagnosis and planning treatment. Weight-bearing X-rays help reveal whether the joint has widened or shifted, and in some cases a CT scan or MRI is used to get a more detailed look at the ligaments and any associated fracture.
How is dual syndesmosis TightRope surgery performed?
The procedure is performed under anesthesia and typically involves small incisions rather than a large open approach. Your surgeon repositions the tibia and fibula into their normal alignment, then places two TightRope devices across the joint to hold the bones securely while still allowing the small, natural motion the joint needs. If there is an associated fracture, it is stabilized as part of the same surgery. Because the implants are designed to stay in place long-term, a separate removal surgery usually is not needed unless a specific problem develops.
What is recovery from dual syndesmosis TightRope surgery like?
Recovery follows a staged process. In the initial weeks after surgery, you will typically be non-weightbearing or limited to protected weightbearing in a boot or cast while the soft tissues begin to heal. As healing progresses, your surgeon will advance you toward full weightbearing, followed by a course of physical therapy to restore strength, range of motion, and balance.
Because every injury and patient is different, your surgeon will give you a personalized timeline for returning to walking, driving, and athletic activity. Following your specific weightbearing and activity restrictions closely gives the ligaments the best chance to heal in the correct position.
What are the benefits of the TightRope technique over traditional screw fixation?
Traditional syndesmosis screws hold the joint rigidly fixed, which can occasionally break or require a second surgery for removal before full weightbearing activity resumes. The TightRope system's flexible design is intended to more closely mimic the ankle's natural, slight motion between the tibia and fibula, and in most cases does not require a planned removal procedure.
