Completing your intake forms ahead of time helps your visit start on schedule. Here's what to know before you fill them out.

How to complete your intake forms

New and returning patients are asked to complete a short intake packet before their appointment. This packet gathers your contact information, insurance details, and health history so your podiatrist has what's needed to provide safe, informed care from your first visit.

There are two forms to complete, both below. The first collects your contact details, insurance, and health history. The second covers the HIPAA privacy notice, our billing policy, and the Advance Beneficiary Notice. Please complete both before your appointment — each one submits separately, so you can do them in either order.

Both forms are hosted and submitted securely by Jotform rather than being stored on this website. If you would rather not complete them online, our front desk team can send paperwork directly to you or help you finish it in person when you arrive.

What you'll need on hand

  • A valid photo ID
  • Your current insurance card
  • A list of current medications and dosages
  • Names of other physicians involved in your care, if applicable
  • Any relevant prior imaging, records, or referral paperwork

What should I bring to my first appointment?

Bring a valid photo ID, your current insurance card, a list of your current medications, and any relevant medical records or imaging related to your foot or ankle concern.

How do I submit my completed forms?

Both forms submit themselves when you press submit at the end — there is nothing to print, sign, or email back. You'll find them on this page. If you'd rather not fill them in online, our front desk team can send paperwork to you or help you complete it in person; just plan to arrive a few minutes early.

Is it safe to submit my medical history through these forms?

The forms on this page are hosted and submitted by Jotform over an encrypted connection, and your answers go to Jotform and our clinic — they are not stored on this website. If you would prefer not to submit health information online at all, call the office nearest you and we will arrange paperwork by another route.

What if I forget to complete my forms beforehand?

That's no problem — arrive a few minutes early and our front desk staff will help you finish your paperwork before your appointment begins.

Do returning patients need to fill out forms again?

Returning patients are typically asked to confirm or update their information rather than complete a full new packet. Call the office nearest you if you're not sure what's needed for your visit.
