What is hallux rigidus?
Hallux rigidus is a condition that causes pain and stiffness in the joint at the base of the big toe. It is a form of degenerative arthritis that develops as the toe's range of motion gradually decreases over time. Because it involves ongoing wear at the joint, it tends to progress in stages, and early recognition allows for more treatment options before the joint becomes severely restricted.
Hallux rigidus is sometimes confused with a bunion because both conditions affect the same joint at the base of the big toe. However, they are different problems — a bunion is a bony bump caused by a shift in joint alignment, while hallux rigidus is a stiffening, arthritic change within the joint itself. What are the stages of hallux rigidus?
Hallux rigidus is a progressive condition. In its earlier stage, sometimes called "hallux limitus," the big toe has limited motion but is not yet completely restricted. As the condition advances toward true hallux rigidus, the joint becomes increasingly stiff, eventually resulting in what is often described as a "frozen joint" with very little remaining motion.
What causes hallux rigidus?
Hallux rigidus develops from a combination of mechanical and structural factors that place extra stress on the big toe joint over time. Contributing factors include:
- Biomechanical issues that alter how weight moves through the big toe joint

What are the symptoms of hallux rigidus?

- Stiffness that worsens in cold or damp weather
- Swelling around the big toe joint
- Difficulty with activities that require pushing off the big toe, such as running or squatting
As hallux rigidus becomes more advanced, additional symptoms can develop, including:
- Pain even at rest, without activity
- Difficulty finding comfortable shoes because of bone spurs forming at the joint
- A change in your walking pattern to avoid bending the toe, which can lead to secondary pain in the knee, hip, or lower back
How do we diagnose hallux rigidus?
Diagnosis is based on a physical examination that checks the range of motion in the big toe joint, along with a review of your symptoms and how they have progressed. X-rays are used to assess the severity of arthritis in the joint, identify any bone spurs, and help determine which stage of hallux rigidus you are experiencing — information that guides the right treatment approach.
How is hallux rigidus treated?
Treatment depends on the stage of the condition, with conservative options generally tried first:
Non-surgical treatment
- Shoe modifications, such as a stiff sole or rocker-bottom design, to reduce the need to bend the toe
