Our Farmington clinic is a full-service foot and ankle practice serving Farmington and the surrounding Davis County communities. To make your first visit smooth, bring your photo ID, insurance card, a list of current medications, and any imaging or records related to your foot or ankle problem. Wearing or bringing the shoes you spend the most time in also helps us assess your gait and footwear. New patients can save time by completing our patient intake forms before arriving.

You’ll find us at 473 W. Bourne Circle, Suite 2, in Farmington, just off Park Lane near the Station Park shopping district and easily reached from I-15 (Exit 325) and the Legacy Parkway. Free surface parking is available directly outside the building, and the entrance and exam rooms are on ground level for easy access. The FrontRunner Farmington Station is only a few minutes away for patients traveling by transit.
