What is Charcot arthropathy?
Charcot arthropathy, also called Charcot foot, is a condition that causes the bones in the foot to weaken in people who have significant nerve damage, or neuropathy. Because neuropathy blunts the ability to feel pain, temperature, and minor trauma, small injuries and repetitive stress can go unnoticed and untreated. The weakened bones can fracture, and continued walking on an already-injured foot allows the joints to shift and collapse, gradually changing the shape of the foot.
Over time, this progression can produce a visibly abnormal foot shape, including a "rocker-bottom" appearance where the arch collapses and the sole becomes convex. Charcot foot is a serious condition that, without early treatment, can lead to severe deformity, chronic wounds, and disability. Diabetes is the most common underlying cause of the neuropathy that leads to Charcot foot, which is why our diabetic foot care program places such an emphasis on daily foot checks.
What are the symptoms of Charcot arthropathy?
Charcot foot often develops with surprisingly little pain, which is part of what makes it dangerous — the same nerve damage that allows the problem to begin also masks the warning signs. Watch for changes in a foot that has known neuropathy, including:
- Warmth in one foot compared to the other
- Redness of the skin over the foot or ankle
- Swelling that develops without an obvious injury
- Pain or soreness, even though sensation may be reduced
- A change in the shape of the foot, such as a flattening or bulging arch
Because these symptoms can resemble an infection or blood clot, any unexplained warmth or swelling in an at-risk foot should be evaluated promptly rather than watched.
What causes Charcot arthropathy, and who is at risk?
Charcot arthropathy develops in people with longstanding peripheral neuropathy, most often from diabetes, which reduces the ability to sense temperature, pain, or trauma to the foot. Without that protective sensation, a person may keep walking on a sprained ankle, stress fracture, or other injury, allowing damage to accumulate unnoticed. Risk factors include:
- Longstanding, poorly controlled diabetes with peripheral neuropathy
- A tight Achilles tendon, which increases stress across the midfoot
- A history of foot or ankle trauma in a foot with reduced sensation
- Continuing to walk or bear weight on an unrecognized injury
- Other conditions that cause significant nerve damage in the feet
Anyone with diabetic neuropathy should be especially alert to the warning signs above, and should have any new swelling, warmth, or shape change in the foot examined right away. Related nerve issues, such as tarsal tunnel syndrome, can also affect sensation and should be evaluated by a specialist.
How do we diagnose Charcot arthropathy?
Diagnosis starts with a careful physical examination, including checking for warmth, swelling, redness, and any change in the foot's shape, along with a review of your neuropathy and diabetes history. Because early Charcot foot can look similar to an infection or a blood clot, we compare both feet side by side and consider your full medical picture.
Imaging plays an important role. X-rays help identify fractures, joint changes, and shifts in bone alignment, and additional imaging such as MRI or bone scans may be used when X-rays are inconclusive or when the diagnosis is uncertain. Catching Charcot foot in its earliest stage, before significant bone collapse occurs, gives us the best chance of preserving the normal shape and function of the foot.
How is Charcot arthropathy treated?
The cornerstone of early Charcot foot treatment is protecting the foot from further damage while the bones stabilize. Because the affected bones are fragile and prone to further collapse, immobilization and reduced weightbearing are essential from the moment the diagnosis is suspected.
Nonsurgical treatment
- Complete immobilization in a total contact cast or specialized boot during the active phase
- Non-weightbearing or reduced weightbearing using crutches, a knee scooter, or a wheelchair
- Close monitoring for signs the bones are stabilizing and inflammation is resolving
- Custom shoes with specialized inserts once the foot has healed, to protect its new shape and reduce pressure points
- Activity modification to avoid repetitive trauma going forward
Healing from an active Charcot episode can take several months to considerably longer, and rushing back to full weightbearing before the bones have stabilized is one of the most common reasons the deformity worsens.
Surgery
Surgery is reserved for feet with severe deformity, instability, or wounds that cannot be managed with bracing and offloading alone. When surgery is appropriate, the goals are to realign and stabilize the foot so it can bear weight safely; the timing of any procedure is determined individually based on how active the disease process still is.
What is recovery from Charcot arthropathy like?
Recovery from an active Charcot episode is a gradual process measured in months, not weeks. You will typically progress in stages — from strict non-weightbearing immobilization, to protected weightbearing in a specialized boot, to custom therapeutic shoes once the bones have fully stabilized. Skipping stages or returning to normal shoes too soon raises the risk of further collapse.
Because Charcot foot tends to recur or affect the opposite foot in people with ongoing neuropathy, long-term follow-up and protective footwear remain important even after the active phase has resolved.
When should you see a podiatrist for Charcot foot symptoms?
If you have diabetes or another cause of neuropathy and notice new warmth, redness, swelling, or a change in the shape of your foot — even without significant pain — seek evaluation right away. Charcot foot is easiest to treat successfully when it is caught before major bone collapse occurs, so do not wait to see if the swelling resolves on its own.
