Sharing your medical history helps us give you safer, more informed care. Here's how we collect it, and why we don't do it through this website.

Why we ask for your medical history

Your medical history — including past conditions, surgeries, medications, and allergies — helps your podiatrist understand the full picture before recommending treatment. This is especially important for patients managing diabetes, circulation issues, or other conditions that can affect foot and ankle health.

How your medical history is collected

To protect your privacy, we do not collect medical history, dates of birth, insurance ID numbers, or other protected health information through our public website. Instead, medical history is gathered as part of your patient intake forms, which are sent through a secure patient portal ahead of your visit or completed on paper in our office.

We don't collect medical history or other protected health information through our public website. It's gathered securely through our patient intake process — either via a secure patient portal or in person at your appointment.

What medical history should I be ready to share?

Be prepared to discuss past surgeries or foot/ankle injuries, chronic conditions such as diabetes or arthritis, current medications and allergies, and any family history relevant to your visit.

Why does my podiatrist need this information?

Your medical history helps your podiatrist choose safe, effective treatment — for example, patients with diabetes or circulation issues often need a different care approach for foot and ankle problems.

Is my health information kept private?
